Grace Hospice is not just for people with end-stage cancer. It can help people with a variety of life limiting conditions which no longer respond to curative treatment. Ask your doctor or contact your local Hospice to learn more about your situation. Patients with wide range of diagnoses (such as cancer, dementia, chronic lung, disease, congestive heart failure, stroke) if disease is advanced, qualify for Hospice care. When appropriate, patient's primary physician and Grace Hospice's Medical Director will authorize start of Hospice care.

Physician Services
Physicians review the patient and family's plan of care and are also available for consultation and home visits.

Patients with limited life expectancy whose
focus is now on physical, emotional, spiritu-
al comfort can apply for hospice care.
When appropriate, patient’s primary physician and Grace Hospice’s Medical Director will authorize start of hospice care.


People with the following illnesses can qualify for hospice care. AIDS, ALS (Lou Gehrig’s Disease) Alzheimer’s Disease or Other Dementias, Cancer, Congestive Heart Disease, End-Stage Liver and Kidney Diseases, Parkinson’s Disease, Lung Disease, Multi-system Breakdown, Multiple Sclerosis and Stroke.

Physicians
Patient’s primary physician may remain as the attending physician. Our experienced Medical Director certifies attending physician’s prognosis, provides home visits as needed, and leads the hospice team in patients’ care. ........................................................

Non-denominational clergy provide spiritual counseling to patients during their hospice stay and bereavement support to their family. ....................................................
